Caveirinha Jiu Jitsu Academy Membership Waiver and Release of Liability
I, the undersigned, hereby apply for Jiu Jitsu instruction through CJJF Central, LLC (“Academy”), which is affiliated with and sanctioned by the Caveirinha Jiu Jitsu Family Association (“Association”). By enrolling in this Academy, I understand that I will be joining the Association and may be eligible to participate in Jiu Jitsu competitions, whether hosted by the Academy, the Association, or other entities.
In consideration of the Academy’s agreement to provide Jiu Jitsu instruction and the associated membership benefits from the Association, I, the undersigned, agree to release, discharge, and hold harmless Caveirinha Jiu Jitsu Academy, its agents, employees, associates, officers, directors, members, volunteers, instructors, participants, affiliates, successors, assigns, and all other persons or entities acting on its behalf (collectively referred to as “Caveirinha Jiu Jitsu Academy”), from any and all liability, claims, demands, actions, causes of action, medical expenses, and any related losses or damages, including death or injury, that I may sustain as a result of my participation in any activities associated with the Academy or the Association.
Assumption of Risk
I understand that Jiu Jitsu is a martial art involving physical contact, including but not limited to chokeholds, arm locks, neck cranks, ankle locks, throws, slams, punches, kicks, elbows, knees, and other techniques that carry inherent risks of serious injury or even death. By participating, I voluntarily assume all risks associated with this form of training and release the Academy from any and all liability resulting from such risks.
I acknowledge that my participation in Jiu Jitsu is purely voluntary, and I choose to participate despite the potential risks.
Health and Emergency Authorization
I certify that I am in good physical condition and know of no medical or physical condition that would prevent me from safely participating in Jiu Jitsu. In the event of an emergency, I authorize any licensed medical professional to provide medical treatment deemed necessary, and I agree to bear any expenses related to such treatment.
Compliance with Academy Rules
I acknowledge that I have received and read the Caveirinha Jiu Jitsu Academy rules, including training and sparring rules. I agree to abide by these rules and understand that any violation may result in suspension or expulsion from the Academy at the discretion of the instructors, with no refund of fees.
Indemnification
Should the Academy incur any attorney's fees, legal costs, or other expenses to enforce this waiver or to defend against any legal claim brought against the Academy by me or on my behalf, I agree to indemnify and hold the Academy harmless for all such fees and costs.
Insurance
I confirm that I either have adequate personal insurance to cover any injury or damage I may suffer during my participation in Jiu Jitsu or that I accept full financial responsibility for any such injury or damage.
Jurisdiction and Governing Law
In the event that I file a lawsuit against Caveirinha Jiu Jitsu Academy, I agree that such action will be filed exclusively in the state of Hawaii. I further agree that Hawaii law will govern the legal action, without regard to the conflict of law rules of any other jurisdiction.
Use of Image and Likeness
I grant the Academy the right to photograph, film, and/or tape my participation in classes, events, or competitions. I understand that these materials may be used for marketing, promotional, or educational purposes by Caveirinha Jiu Jitsu Academy, and I waive any right to compensation for such use.
Acknowledgement of Understanding
By signing this release, I confirm that I have read and understand the contents of this waiver and voluntarily agree to its terms. I acknowledge that I am over 18 years of age and of sound mind. If I am under 18, my parent or legal guardian must sign below, accepting the terms and conditions of this release on my behalf.

I, the undersigned, represent that I am the parent or legal guardian of [Minor's Name] (“Minor”), and in consideration of Minor being permitted by Caveirinha Jiu Jitsu Academy, CJJF Central, LLC, and/or Caveirinha Jiu Jitsu Family Association to participate in its activities, I agree to indemnify and hold harmless Caveirinha Jiu Jitsu Academy, CJJF Central, LLC, and the Caveirinha Jiu Jitsu Family Association from any and all claims brought by or on behalf of Minor in connection with such participation.
